So your fundamentals are sound. You can shoot a tight group on demand. You've been practicing a lot too. But you're still missing, and have no idea why.
It's pretty common. If you practice (either dry fire or live fire) on the same target array for too long you develop a rhythm. Look at the splits on the timer, I bet you'll find that they're very similar. When you shoot a target array that's different from your practice array you find yourself missing without knowing why. It's because you're still shooting that practice array.
The rhythm that you developed is a result of training the subconsious to shoot one particular array instead of shooting to the sights. You start pulling the trigger when the timing of the sweep is right and not when you have a sight picture.
So how to break that habit? You have to mix it up. Different sized targets, targets at variable distances and heights, some close together and some spread out. Don't shoot the same array more than a few times.
Mix up your live fire targets, mix up your dry fire targets. Shoot some groups again.
And get back to your sights!
